Patellar luxation—often called a “loose kneecap”—is one of the most common knee problems in dogs, especially small and toy breeds. It happens when the kneecap slips out of its normal groove, causing your pet to skip, hop, limp, or show signs of knee pain. In more severe cases, the kneecap may stay out of place, leading to long-term discomfort, arthritis, and an abnormal gait.

While some mild cases can be managed with soft tissue procedures alone, many dogs develop luxation because the bones of the leg are not properly aligned. When this happens, a corrective osteotomy (a controlled surgical cut in the bone to realign the limb) is needed to keep the kneecap in place permanently.

This surgery corrects the root cause of the luxation, restores normal knee movement, and greatly reduces the risk of the kneecap slipping again.


Why Corrective Osteotomy Is Sometimes Necessary

In many patients, especially those with moderate to severe luxation (Grade III–IV), the issue is not just a loose kneecap but a misalignment of the entire leg. This may include:

  • The tibial tuberosity (where the kneecap’s tendon attaches) being too far inward or outward

  • A bowed or angled femur

  • Rotational twist of the femur or tibia

  • A shallow groove that does not hold the kneecap properly

If these bony issues are not corrected, the kneecap will simply continue to slip—even after soft-tissue repair. Corrective osteotomy ensures the leg’s alignment is restored, allowing normal tracking of the kneecap.


How the Surgery Works

Corrective osteotomy is a highly specialized orthopedic procedure. Depending on your pet’s unique knee shape and the degree of deformity, your surgeon may perform:

Tibial Tuberosity Transposition (TTT)

Realigns the attachment of the kneecap tendon so the kneecap tracks straight.

Distal Femoral Osteotomy (DFO)

Corrects bowing or angulation of the femur to restore the mechanical axis of the leg.

Femoral or Tibial Rotational Osteotomy

Used when the limb has excessive inward or outward rotation that pushes the kneecap off track.

In many cases, the surgeon will also deepen the kneecap groove (trochlear recession) and tighten or release surrounding tissues to balance the knee.


Recovery and Home Care

Healing usually takes 8–12 weeks, depending on the type of osteotomy performed.

What to expect:

  • Your pet will need strict rest at home

  • Only short leash walks for bathroom breaks

  • No running, jumping, or stairs

  • An Elizabethan collar (cone) to prevent licking

  • Twice-daily incision checks for redness, swelling, or discharge

Follow-up X-rays will be needed at 6–10 weeks to ensure the bone is healing properly.


Long-Term Outcome

Corrective osteotomy offers an excellent prognosis. Most dogs return to a comfortable, active lifestyle once the bone has fully healed, and recurrence rates are low when alignment is corrected properly. Rehab exercises and hydrotherapy can further improve recovery.
