The carpus (wrist) is a complex joint made up of several small bones and strong ligaments that help your dog walk, run, turn, and bear weight comfortably. When this joint is severely damaged—whether from a traumatic accident, a fall, a high-impact injury, or advanced arthritis—it can become unstable and painful. In many cases, the damage cannot be repaired with simple ligament repair or internal plating alone.

Pancarpal arthrodesis, also known as panarthrodesis, is a specialized surgical procedure designed to fuse all levels of the carpal joint. While this removes normal motion in the wrist, it creates a strong, pain-free limb that allows your dog to walk comfortably again.


When Pancarpal Arthrodesis Is Recommended

Your veterinarian may recommend this procedure if your dog has:

  • Severe hyperextension of the wrist

  • Complete dislocation (luxation) of the carpal joint

  • Comminuted or irreparable carpal fractures

  • Severe shearing injuries, often seen in road traffic accidents

  • End-stage arthritis

  • Chronic instability that is no longer manageable with bracing or medication

When the supporting ligaments or bone structures are destroyed, the carpus becomes unstable and very painful. In these cases, fusion is the most reliable way to restore stability and comfort.


How the Procedure Works

The goal of pancarpal arthrodesis is to create a permanently stable wrist so your dog can use the limb without pain.

1. Preparing the Joint for Fusion

The damaged joint surfaces are cleaned, and the remaining cartilage is removed to encourage the bones to fuse. A small amount of your dog’s own cancellous bone (bone graft) is placed into the area to promote healing.

2. Stabilizing the Carpus

A specialized plate is then placed on either the top (dorsal) or underside (palmar) of the limb and secured with screws. This plate holds the joint in a functional angle—usually around 10–12 degrees of extension—to mimic a natural standing posture.

In some severe wounds or contaminated injuries, an external fixator may be used instead of a plate.


What to Expect After Surgery

Your dog’s comfort and recovery depend heavily on strict home care and follow-up visits.

Activity Restriction

Your dog will need strict rest for 6–8 weeks. This means:

  • No running or jumping

  • Leash walks only for toileting

  • Confinement to a crate or small room when unsupervised

Bandaging and External Support

A bandage, splint, or cast is often required for several weeks to protect the surgical site until early fusion occurs.

Incision Care

Check the incision twice daily for:

  • Swelling

  • Redness

  • Discharge

  • Foul odor

Use an Elizabethan collar at all times to prevent licking.

Rehabilitation

Once the bone starts to fuse, gentle strengthening exercises and hydrotherapy may be recommended.

Most dogs show improvement in limb function within a few months and return to a comfortable, active lifestyle.


Expected Outcome

Although your dog will lose wrist movement, most pets adapt extremely well. The limb functions reliably, pain is significantly reduced, and most dogs return to normal daily activities.
